Application Forms

These application forms are available in both MS Word and Adobe Acrobat PDF format. To download them, click on the format you prefer and save them to your computer. You can then print them and fill them out at your convenience.

If you are seeking admission for more than one child, you can include both of them on the "Application for Twin Enrollment" form, and simply include a separate student evaluation form and school information request form for each.

A complete application package consists of:

  • the appropriate application form for your child or children
  • the appropriate student evaluation form(s) for the grade(s) each child will enter
  • a School Information Request form for each child
  • a check for the non-refundable $75 application fee.
